Parental Waiver and Release

We are pleased to learn that your child is interested in participating in our Summer Youth webinar program, “B’Tweens”. This program is not a replacement of adult supervision of the children participating, but we will make every effort to provide engaging and age appropriate activities during the scheduled program hours.

Given the digital format for the summer program, we anticipate that some of the activities will include creation of videos and social medial content that is age appropriate, and which we will monitor. Unless we hear from you otherwise, we will assume that we have your consent to share any such productions on social media channels and for promotional purposes. We will take all reasonable precautions to protect the privacy, identity and confidentiality of all of the youth participants and will alert you if there are any known breaches in our secure Zoom platform.

As the parent, or legal guardian, of the child named below, I hereby give my full consent and approval for my child to participate as a team member in the B’Tweens Summer Program. In addition to giving my full consent for my child’s participation, I do hereby waive, release and hold harmless the organization named below, its officers, board members, sponsors, supervisors and representatives during my child’s participation in activities, whether the result of negligence or any other cause.

תוכנית זו הינה תוכנית מקוונת שמאורגנת על-ידי בני נוער, לנוער. אנו נעשה את מירב המאמצים לוודא שהפעילויות יותאמו לגיל המשתתפים. הפעילויות יהיו תחת השגחת מבוגרים. עם זאת, האופי המקוון של התוכנית אינו מהווה תחליף לפיקוח של מבוגר בפועל

אנו נשתמש בטכנולוגיית שיחת ועידה מאובטחת, סגורה, פרטית של זום לפלטפורמה של הפעילויות. אנו עוקבים מקרוב אחר כל המשתתפים, וננקוט בכל הצעדים הסבירים בכדי למנוע גישה בלתי מורשית לפעילויות. בהתחשב בפורמט הדיגיטלי לתכנית הקיץ, אנו צופים כי חלק מהפעילויות יכללו יצירת סרטונים ותוכן מדיאלי חברתי המתאים לגיל. אנחנו לא נפיץ את החומר, לא נמכור אותו או נעביר אותו לידי צד שלישי. חתימתך על טופס זה מציינת שאתם מסכימים שנשתמש בתוכן בדיגיטאלי למטרות קידום ושיווק שלנו בלבד. אנו ננקוט בכל אמצעי הזהירות הסבירים לשמירה על פרטיותם, זהותם וסודיותם של כל משתתפי הנוער ונתריע בפניכם אם יש הפרות ידועות

על ידי אישור מסמך זה, אתם מאשרים כי, כהורה, או כאפוטרופוס חוקי, של הילד המופיע למטה, אתם נותנים בזאת את הסכמתכם המלאה ואת אישורכם לילדכם להשתתף בפעילות כמתואר לעיל
